Description/What’s Included:
- Comprehensive, initial evaluation to assess client’s mental health history, presenting symptoms, and overall psychological well-being.
- Risk assessments (suicidality, self-harm, etc.).
- The evaluation includes a detailed interview, review of medical and psychiatric history, family history, social history, and any past treatments or medications.
- Review of current medications and treatment plans.
- Standardized assessment tools and diagnostic criteria are utilized to accurately diagnose mental health conditions.
- Initial treatment recommendations (e.g., medications, therapy options).
- Following the assessment, a personalized treatment plan is developed in collaboration with the client.

POLICIES
Telehealth Information
Purpose of Telehealth:
Telehealth involves the delivery of psychiatric and wellness services using
secure, HIPAA-compliant video conferencing technology. It allows you to receive care from the comfort of your own home or private location.
- Confidentiality & Privacy:
Your telehealth sessions are conducted over a secure platform, and your privacy is protected in accordance with HIPAA guidelines.
You are responsible for ensuring you are in a private location during sessions.
Benefits of Telehealth:
- Improved access to care
- Reduced need for travel and time off
- Increased flexibility
Limitations of Telehealth:
- Technical difficulties (e.g., connectivity issues) may interrupt or delay sessions.
- In some cases, physical exams or lab work may still be required in-person.
Emergency Situations:
- Telehealth is not appropriate for emergency care. If you are in crisis, please call 911 or go to your nearest emergency room.
- The National Suicide & Crisis Lifeline can be reached at 988.
Location Disclosure:
- You must physically be located in one of the states where services are offered (Alabama, Arizona, Colorado, Idaho, Oregon, or Washington) at the time of each telehealth session.
Technology Requirements:
A reliable internet connection, camera, and private space are necessary for all
sessions.
Privacy Policy & HIPAA Acknowledgment
This notice describes how your health information may be used and
disclosed, and how you can access this information. Please review it carefully.
Your Rights:
- You have the right to access and request copies of your medical records.
- You may request corrections to your records if you believe information is incorrect.
- You may request confidential communications.
- You may request a list of disclosures of your health information.
How We Use Your Information:
Your personal health information may be used for the following:
- Treatment planning and coordination
- Billing and insurance purposes
- Healthcare operations and quality improvement
- Legal requirements (e.g., reporting abuse, threats of harm, or as required by law)

We maintain strict physical, electronic, and administrative safeguards to

Payment Policy
Payment Expectations:
- Self-pay clients: Payment is due at the time of service.
- Insurance clients (Alabama only): We will bill your insurance. You are responsible for any co-pays, deductibles, or non-covered services.
- Accepted Insurance: Aetna, BCBS, Cigna, Medicaid, Medicare, United Healthcare (Alabama clients only).
- Accepted Payments: Credit/Debit cards, HSA/FSA, and electronic payments.

Missed Appointments:
- We require at least 24 hours’ notice for cancellations.
- No-shows or late cancellations may incur a fee of $50. Insurance does not cover missed appointments.
Refund Policy:
All payments are non-refundable once the session is completed.
No-Show and Late Cancellation Policy
Purpose
This policy is designed to promote accountability and ensure availability
for all patients seeking care. Missed appointments reduce our ability to serve
others and may interrupt the continuity of your treatment.
Appointment Cancellations
We require at least 24 hours’ notice to cancel or reschedule any
appointment. You may cancel by calling or messaging the office directly.
Late Cancellations
Cancellations made less than 24 hours before your scheduled
appointment will be considered a late cancellation.
No-Shows
A "no-show" occurs when:
- You miss your appointment without any prior notice
- You are more than 10 minutes late to a 30-minute appointment or 15 minutes late to a 60-minute appointment without notice
- If you arrive more than 10 minutes late to a 30-minute appointment or more than 15 minutes late to a 60-minute appointment without prior notice, your appointment will need to be rescheduled and will be subject to a late cancellation fee.
Fees
- Late Cancellation or No-Show Fee: $50.00
- This fee is not covered by insurance and must be paid before your next appointment can be scheduled.
Repeated Missed Appointments
Frequent no-shows or late cancellations (3 or more within a 6-month
period) may result in:
- Discontinuation of services
- Being referred to another provider better suited for your scheduling needs
Controlled Substance Policy
Calming Reflections Psychiatry LLC
Maintaining safe, ethical, and compliant prescribing practices
Purpose
This policy outlines the standards and responsibilities associated with
prescribing and managing controlled substances (e.g., stimulants,
benzodiazepines, and certain mood stabilizers) at Calming Reflections
Psychiatry LLC. Controlled substances are prescribed only when
clinically appropriate, and only in accordance with state and federal laws.
Key Guidelines
1. Initial Evaluation Requirement
Controlled substances will not be prescribed during the initial
consultation. A comprehensive psychiatric evaluation and review of medical
history is required before any decision is made regarding controlled substance treatment.
2. Treatment Agreement
If controlled substances are deemed appropriate for your treatment:
- A Controlled Substance Agreement must be signed.
- You may be asked to undergo periodic urine drug screening.
- Ongoing prescriptions are contingent on regular follow-ups and full compliance with treatment recommendations.
3. One Prescriber Rule
You must receive controlled substances only from Calming Reflections
Psychiatry LLC unless otherwise authorized. Obtaining medications from
multiple providers without disclosure may result in immediate termination of
care.
4. Medication Monitoring and Refills
- Lost or stolen prescriptions will not be replaced.
- Early refills will only be considered for documented emergencies.
- Refills are not guaranteed and may be denied if there is:
- Non-adherence to the treatment plan
- Concerning behavior or signs of misuse
- Missed follow-up appointments
5. State Prescription Monitoring
Programs (PMPs)
We routinely review your medication history using state Prescription
Monitoring Programs to ensure safe prescribing and prevent misuse or
duplication.
6. Prohibited Behaviors
The following may result in discontinuation of controlled substances
and/or termination of care:
- Sharing, selling, or altering medication
- Using illicit substances (including non-prescribed cannabis where prohibited by law)
- Doctor shopping
- Frequent medication “emergencies” or inconsistencies in medication use
- Aggressive or threatening behavior toward staff regarding prescriptions
Patient Responsibilities
- Take medication only as prescribed.
- Attend all scheduled follow-up appointments.
- Safely store your medication.
- Notify us of any side effects or concerns immediately.
Inform your provider of any new medications prescribed by other doctors.
